  1. Due to legal reasons and insurance carriers' requirements, a standard
    Chapter Membership and Release Form must be used.

  1. Print out and fill in the 2 page MS-Word document  Chapter Membership Enrollment and Release Form. You may need to download the Adobe Reader program in order to view this file. This program can be downloaded from GetAcrobat

  2. Chapter Name - Sauk Prairie H.O.G. Chapter #5626
     
  3. Member Name - (One member per form only! - Please make necessary copies)
     
  4. Address/City/State/Zip - Required.
     
  5. Email address, Phone Number, and Birthdate - Optional.
     
  6. National H.O.G. Number and Expiration Date - Application will not be accepted without these. No Exceptions! Each applicant must have their own National H.O.G. number and can NOT use a spouse's or parent's National H.O.G. number.
     
  7. You are signing that you have read the Annual Charter for H.O.G. Chapters. Here's a link.
     
  8. Newsletter Delivery: USMail - hard copy, Email - link each month, Website - web browser.
     
  9. Member Signature - Required.
     
  10. Local Dues - $2000 per family. One newsletter sent per family.
